Foreskin inflammation

Asked for Male, 31 Years

Married with one child in a monogamous relationship. I had severe chemical reaction on my foreskin 4 years ago which damaged my foreskin. It was inflamed and stayed like that for almost 3 years. Also Had changes in my urine flow and still same to date, slow urine stream with some dribble in the end. Masturbated a few days ago and now foreskin is swollen again with pain and pain during urination. Swollen veins in foreskin and foreskin has become very tight. Please advise. Thanks
