This sounds like chronic diarrhea that is probably unrelated to the fistula surgery itself and now needs a structured work‑up rather than repeated short antibiotic courses. Chronic diarrhea is defined as loose or soft stools for more than 4 weeks and is commonly due to irritable bowel syndrome, post‑infectious bowel dysfunction, inflammatory bowel disease, malabsorption, microscopic colitis, or medication/laxative overuse etc In patients with a past anal fistula, Crohn’s disease should be kept in mind; a minority of patients with fistula later turn out to have underlying IBD, especially if they have chronic diarrhea, weight loss, or abdominal pain.
