Start with:
Warm Sietz bath with KMnO4 powder (2 pinch) BD for 20 mins.
Syp. Lactulose (Emty) 30ml H.S.
Ointment Anosum 2-3times per day
Ointment Hemolit 2-3times per day
Since you had surgery two times in recent past you need a thorough clinical examination to rule out other causes of perianal burning pain beside fissure in ano. Fidonal ointment can be applied thrice daily
