Fever not came down

Asked for Female, 25 Years

My baby has fever from thursday night till now the fever has not came down we went to doc he prescribed cefalexin and meftal and then if fever came he told to puy paracetamol.. if we use tablet the fever came down. Can you please suggest mw wat to do. He s 3 years old and weight is 12.50kg. He s nt having vomiting diarhoea cold cough no rashes nting but he has chills after putting tablet. I think it might be typhoid. Attached image if this is tongue coaated with fur.
