I am a 27-year-old male with rapid weight gain despite a low-carbohydrate diet (about 5 chapatis/day and one bowl of sabzi). I am a non-smoker and non-alcoholic. My reports show markedly elevated liver enzymes (ALT 263, AST 110, GGT 142), suggesting possible liver damage or fatty liver. I also have high uric acid (8.5 mg/dL) with knee pain and gout symptoms, severe vitamin D deficiency (4.4 ng/mL), and low-normal vitamin B12 (240 pg/mL). Lipids and hs-CRP are elevated. I get significant cough on smoky days.

Febustat 40 once a day after dinner x 4 weeks for high uric acid. Alt and ast along with ggt are increases due to various factors. As you mentioned you are non alcoholic, then it is non alcoholic fatty liver or might be due to various medications taken in the past.

Alcohol is not only the reason behind Elevated liver enzymes , you are non alcoholic, but there is fatty liver problem due to other reasons also like carbohydrates rich diet,less excercise or sedentary lifestyles. D3 deficiency also, so you need to evaluate first and then start proper treatment, please consult for further management and evaluation.

Do the following, 1. Blood pressure check 2. At least 75minutes of brisk walking or jogging a week 3. Blood tests : HbA1C, TSH, Hepatitis B, Hepatitis C,Ferritin, Ceruloplasmin, INR, CBP 4. Ultrasound abdomen and Fibroscan of Liver 5. Have daily Protein at least 1gm per kg body weight and avoid painkillers like Ibuprofen etc 6. Medications i) Allopurinol 100mg once a day for Gout ii) Rosuvastatin 10mg once a day for high lipids iii) Cholecalciferol 60k once a week for 8 weeks and then once a month for a year Medications will need to be taken regularly and based on follow up blood tests need dose adjusting. 7. Reports mein ALT/AST/GGT ka level kaafi elevated hai jo most commonly fatty liver (NAFLD) + metabolic inflammation se related hota hai, especially rapid weight gain aur high lipids/uric acid ke saath. Ye alcohol se nahi bhi hota — diet pattern aur insulin resistance se bhi hota hai. Filhal liver ko rest dena important hai: avoid sugary drinks, fructose, packaged snacks, red meat excess aur late night meals. Daily 30–40 min brisk walk aur 7–8% weight reduction se enzymes significantly improve hote hain. High uric acid gout symptoms de raha hai — hydration >3L/day rakhein aur high-purine foods (beer, organ meat, excessive dal supplements) kam karein. Medicine decision symptoms frequency dekhkar liya jata hai, har report pe immediately start nahi karte. Severe vitamin D deficiency treatment required hai (weekly supplementation) aur B12 borderline hai.
