Asked for Male, 2 Years

My baby is two years of old having spina bifida. Undergone two surgeries. Now my issue is my baby is having dry lips. Cracks were appearing everyday over the lips. We had applied butter and ghee but there is no change. We initially thought it's due to dehydration and given water but there is no change . I had attached image. Plz give me some solution and the reason for coming this. Reply ASAP.
