In 2024 my ear crystals got displaced and after that I started having very frequent dizziness, vertigo and vomiting especially when I go to the gym and when I drive even if I am sitting in the passenger seat. I am really very disturbed by this dizziness. Because of this anxiety has also started. Many times I feel off balance like my brain is not in my control. That is why I first got a brain MRI done which came normal. After that I got a cervical and full spine MRI done whose images I had shared. Kindly help me I am really in need of it.

Your symptoms fit very well with persistent vestibular dysfunction after BPPV (ear crystal displacement), now worsened by cervical spine involvement and anxiety.
Key points to understand clearly:
• Your brain MRI is normal → this rules out dangerous brain causes
• Your MRI shows C3–4 and C4–5 left-sided disc protrusion with nerve root irritation → this can cause neck-related dizziness (cervicogenic dizziness), head pressure, imbalance, and nausea
• After BPPV, some people develop persistent dizziness even when crystals settle — the brain remains “over-alert” to motion
• Anxiety then amplifies dizziness, creating a vicious cycle
Importantly:
Next Steps
You need a targeted, multi-directional approach, not random treatment.
1. Specialist to Consult
• ENT (Otoneurologist) or Neurologist with vestibular experience
• Physiotherapist trained in vestibular + cervical rehab
2. Required Treatments
• Vestibular rehabilitation therapy (VRT) — not just one maneuver
• Cervical spine physiotherapy (posture correction, deep neck flexors)
• Temporary avoidance of heavy gym workouts (especially weights, neck strain)
• Medication only if needed for vestibular suppression or anxiety (short-term)
3. What NOT to Do
• Repeated MRIs
• Excessive Google searching
• Avoidance of all movement (this worsens recovery)
• Self-neck manipulation or forceful exercises
Health Tips
• Stop gym activities involving neck loading or jerky movements
• Gentle walking is encouraged
• Use a proper cervical pillow
• Avoid prolonged phone bending
• Practice slow breathing during dizziness (reduces vestibular–anxiety loop)
• Limit caffeine and screens temporarily
• Your symptoms are real, but not dangerous
• Many patients recover fully with the right therapy
• Anxiety is a result, not the cause — and it will reduce as dizziness improves
• You do not have brain disease or loss of control

The changes on the scan have nothing to do with your symptoms which sound like BPPV.
Do the following
1. Vestibular exercises- video here how to do

2. Avoid medications including Betahistine, Prochlorperazine, Cinnarazine etc. They don’t fix the problem but may briefly reduce symptoms
All the best.

Doing day to day activities with these symptoms must be hard for you, especially driving which is risky both ways.
Next Steps
visit an ENT Specialist. There is a specific maneuver that is done to relocate the displaced crystals which will most probably give you an immediate relief,
however the crystal may dislodge again as the reoccurrence rate is high.
Health Tips
would recommend you to take one attendant with you and learn the maneuver from the certified doctor itself. May help you in future recurrences and save you some money.
Your problem is curable. Consult a neurosurgeon for surgical correction.
Till then, take tab vertin 16mg one tab. morning and evening for a week.
Tab promethazine 25mg every 8 hours for 5 days.
Avoid neck bending, using high pillow.
