First of all we need to confirm that are we working on a correct diagnosis? Is it still depression or other things have taken a toll?
Treatment options have progressed a lot over these last few years. A holistic approach combining medicines along with an integrative therapy along with modified lifestyle might give you best results rather than one type of treatment alone. Your confidence level seems to have gone down with whatever has been happening in your life and individual therapy would help you a lot to gain that back.
Next Steps
Consult a psychiatrist along with a psychotherapist, go for collaborative treatment.
