Creatinine fluctuations are common when lasix or zynatix are used as they increase urine production by straining kidneys . Heart failure itself is a stress for kidney
Next Steps
Keep on monitoring creatinine

When ever there is challenge to a damaged kidney- in this case fluid overload there will increase in creatinine. Usually 0.3mg increase is considered significant but in chronic cases we see fluctuations in creatinine 0.5 due to various factors like fluid intake salt intake exercise protein intake etc. he also has loss of protein in urine. I would suggest to consult nephrologist for the possibility of renal biopsy to give accurate diagnosis as to why the creatinine is fluctuating and protein loss in urine
