Hi... as per your saying it seems to be a long standing infection. We need do an xray for that particular tooth and evaluate the lesion present around your tooth roots. Charges vary depending upon the extent of the lesion and the amount of tooth decayed since its been decayed for 5 yrs and the type of crown u want put on..
