I think your gynaecologist  have not told you how milk production  start, you only do one thing that child should suck the nipple at regular time so that there is milk let down reflex will start and child should suck when hungry  not when his stomach is filled with top milk, you should drink milk, and water regularly and check that the holes are open or not by squeezing the nipple, for medicine you chat me online on Practo consult app
