Hello, I am Dr. Romain Rajan, General Physician, and your case has been allotted to me. I understand that seeing blood in phlegm can be very frightening, especially when it happens suddenly. Please try not to panic. Based on what you have described, this does not sound like an emergency, and in many cases the cause is treatable and benign. What could be the reason Since the blood appears only in the morning, after brushing and retching, and you have a history of smoking, coughing, and recent exposure to pollution (Delhi), the common causes include: • Irritation or small surface injury in the throat due to forceful coughing or retching • Bleeding from gums or oral cavity aggravated by brushing • Inflamed throat or bronchitis, very common in smokers and after pollution exposure • Acid reflux (GERD) causing morning nausea and throat irritation • Old, stagnant phlegm mixed with a small amount of blood collected overnight True lung-related bleeding usually occurs throughout the day and is often associated with large amounts of blood, which you have not described. What you should do now • Avoid forceful retching and aggressive brushing • Use a soft toothbrush • Drink warm water on waking up • Continue complete abstinence from smoking and alcohol • Avoid spicy, oily, and late-night meals • Observe whether the blood is just streaks or in larger quantity When to consult a doctor immediately • If the amount of blood increases • If blood appears multiple times during the day • If you develop fever, weight loss, night sweats • If you have chest pain, breathlessness, or worsening cough • If this problem continues beyond 3–5 days A doctor may advise basic blood tests, a chest X-ray, or ENT/chest evaluation if required. About informing your parents I understand your hesitation, but early medical evaluation is always better and prevents unnecessary complications. From your description, this does not point toward a serious condition, but it does require proper assessment. You have taken a very positive step by stopping smoking and drinking. For a one-to-one consultation, you may reach out to me on 85271646 seven seven. Feel free to share any reports or further symptoms. I am here to guide you.

Blood in phlegm only in the morning with retching is often due to throat irritation, acidity, or forceful coughing, especially with smoking history. Small streaks can happen and are usually not serious.
Next Steps
Still, get checked by a doctor for safety. Avoid smoking, alcohol, and spicy food. If bleeding increases or happens throughout the day, seek care urgently.
