How common is it for major blood vessels and arteries to be cut or damaged during Fess or Septoplasty Surgery. Is it very difficult to stop?

In my career of 13 years, I have seen major bleeding happen 3 times, assuming 1000 cases, 0.03%, 1 case we had a major bleeding after FESS and unable to identify the cause itself as he used to bleed torrentially in home and stopped on reaching hospital, we would not find any bleeding, happened again and again, everything we see nothing,  everytime discharge go home, massive bleeding soaking pillow, bedsheets, which he showed as proof.
