This is most commonly due to anal fissure or piles, usually caused by constipation. It is common and treatable, not serious in most cases.
Rx:
Syp Cremaffin Plus 10–15 ml HS
Isabgol 2 tsp HS with warm water
Lignocaine + Sucralfate ointment local, before & after stools
Warm sitz bath BD
Fluids + fibre rich diet
Consult a Surgeon for evaluation and management.
If you have history of hard stools , with low water intake and low fiber , this is probably due to piles or small anal fissures due to excess stretch of anus while passing stools . Try drinking 2-2.5L water with plenty of salads . If this condition pertains with pain , better consult a general surgeon .
Seeing blood while using the toilet can be scary, especially at your age, but most of the time it is not serious. In teenagers, this usually happens because of hard stools, straining, or a small tear at the opening, especially if there is constipation. If the blood is fresh red, small in amount, and happens only during passing stools, it is usually not dangerous. If you want, we can also talk about this in an online consultation so I can understand it better and reassure you properly
Next Steps
Try to notice if the stools are hard or if you had to strain.usually we manage this with medicines from stool-softening or bowel-regulating drug classes rather than strong treatments. If bleeding keeps happening, becomes heavy, or comes even without stools, you should see a doctor for examination.
Health Tips
Drink plenty of water and eat more fruits and vegetables to keep stools soft. Avoid straining or sitting too long in the toilet. Seek medical help urgently if there is severe pain, dizziness, black stools, or ongoing bleeding.
If this happened for the first time and was painless, better to keep a watch at it, if it happens again, probably it is piles, mostly due to constipation, low fiber intake.
Next Steps
Book an online consultation for detailed review.

Seeing blood from the anus during toilet in a 15-year-old is most commonly NOT serious. In this age group, the usual cause is:
Most common reasons
• Hard stool / constipation causing a small cut at the anus
• Straining during toilet
• Sometimes mild piles due to constipation
Bright red blood seen on stool or on toilet paper, with or without pain, usually points to a local anal cause, not an internal disease.
What to do now (without medicines):
• Drink plenty of water
• Eat high-fiber foods: fruits, vegetables, salads
• Avoid junk food and spicy food
• Do not strain or sit too long in the toilet
• Maintain regular toilet timing
When to worry and seek urgent care:
• Blood continues daily
• Severe pain, dizziness, or weakness
• Black stools, fever, or weight loss
Most cases settle once constipation is corrected.
