Hi sir...
Bad breathe is due to poor oral hygiene like soft and hard deposits(plaque and calculus) which cause gum infection(gingivitis)leads to bleeding gums which progressively affects supporting tissues of tooth (periodontitis) ,decayed tooth....even though you brush there is always some inaccesible areas in our teeth ,in that area food sticks and cause bad breathe that need to be cleaned (scaling) by professional dentist ....
Next Steps
Kindly visit nearby dentist for furthur consultation and treatment
Health Tips
Do scaling(cleaning) for every 6 months and use mouth wash regularly ....

Only brushing is not enough. Firstly get your scaling and polishing ( cleaning of teeth) done. If the problem is more severe you may need further periodontal treatment. After that Home maintainence with brushing, flossing and mouthwash.
Health Tips
Other causes of bad breath besides oral hygiene should also be ruled out
