Anxiety and Panic Episodes
I am experiencing frequent anxiety and panic episodes. During these episodes I feel chest heaviness, difficulty breathing, trembling in hands and legs, dizziness and sometimes fainting. My blood pressure drops during these situations. I also have a history of low hemoglobin. I want to understand my mental health condition and get proper medical guidance.

Hi What you’re experiencing chest heaviness, breathlessness, trembling, dizziness, and fainting can be part of panic or anxiety episodes, where the body goes into an intense “fight or flight” response, but since you also have low hemoglobin and report blood pressure drops, it’s important not to assume it’s only psychological. First, rule out physical causes with a physician (check hemoglobin, BP, and overall health), because anemia itself can cause weakness, dizziness, and fainting. At the same time, anxiety can amplify these sensations, creating a cycle of fear and physical symptoms. Start with grounding and breathing techniques (slow exhale-focused breathing), avoid sudden standing, stay hydrated, and notice the thoughts that come before episodes to reduce panic escalation. Since your symptoms are intense and recurring, a combined medical and psychological approach will help you manage this safely and effectively. Take therapy. I want to acknowledge how much courage it takes to voice these struggles. Often, we treat mental health as a matter of 'willpower,' but as a psychiatrist, I can tell you that what you are experiencing has a real biological and neurological basis. Your brain, like any other organ, can sometimes struggle to maintain balance under stress. Seeking treatment isn't a sign of failure; it’s a proactive medical decision to regain your quality of life. We can work together to understand the 'why' behind these feelings and find a structured path back to your baseline. You don't have to navigate this alone.

The symptoms you’ve described—chest heaviness, trembling, and difficulty breathing—are classic signs of Panic Disorder or Severe Anxiety. However, because you mentioned fainting, dizziness, and dropping blood pressure, we must also consider your low hemoglobin. Anemia can cause physical weakness and heart palpitations that mimic or trigger panic attacks. When your brain doesn't receive enough oxygen due to low hemoglobin, it can trigger a physical "alarm" that feels exactly like anxiety.
Next Steps
Please get a fresh Complete Blood Count (CBC) test to check your current hemoglobin levels. It is vital to rule out physical exhaustion from anemia as a cause for your fainting spells. When you feel an episode starting, sit or lie down immediately to prevent injury from fainting or a drop in blood pressure.
Health Tips
You should consult with a psychiatrist to discuss whether anti-anxiety medication or therapy is needed to manage the panic episodes.
