Hi there...
I'm guessing you are fed up with this recurring acne and want a clear face.
My suggestion is to go for some oral (systemic) therapy with simple Vit A derivative. Yes there are some basic side effects like dryness of 👄, 👃 etc that can be easily tackled with application of vaseline and good hydration 💦
A little more information about your menstrual cycle, any Pcos & chances of conception needs to be ascertained and once all this is done, can prescribe the medication and it will take anywhere between 3-6 months for a near total clearance.

Do you have flaky dandruff over scalp? This might trigger acne over forehead, also using any product for hair like oil/hair gel?

Your description is most consistent with mild inflammatory acne vulgaris limited to the forehead, with recurrence after stopping an effective agent rather than treatment failure
Next Steps
consult online with me for further evaluation and management
• Likely recurrent comedonal + inflammatory acne on forehead (T-zone oil related)
• Flare-ups after stopping treatment suggest maintenance not continued
• Humidity, sweat, hair products or dandruff may be triggering forehead acne
Next steps
• Continue oil-free moisturizer (Excela is fine)
• Add anti-dandruff shampoo (ketoconazole) if dandruff present (2–3x/week)
• Avoid heavy hair oils/creams touching forehead
• Give treatment 6–8 weeks consistently
Helpful tips / Words of caution
• Avoid over-washing or harsh scrubbing
• Keep hair off forehead, clean pillowcases regularly
• Use non-comedogenic sunscreen daily
