My father is on AKURIT-4 yesterday was his first dose. He is weighing 56 kg and he's having like itching yesterday the eating was limited to his hands, but today his whole body is itching. I have given him MonteLC which subsided the itch for sometime but again it has mildly started is it a matter of concerned because he had like a hepatitis B positive, but it is inactive and his left is preferably normal before starting the therapy Today day to therapy what should be the next plan of action?

Hi, itching after the first dose can be a reaction to the medicines.
Since it responded to Montelukast, continue. Stop the medicine temporarily if it worsens and inform the treating doctor immediately.
Feel free to consult for further guidance. 👍

Whole body itching after starting Akurit-4 (anti-TB medication) is a recognised side effect and should be taken seriously, especially since it has progressed from hands to the whole body by day 2. This kind of generalised itching can be a drug hypersensitivity reaction, and with his Hepatitis B positive status, liver monitoring is also important as anti-TB drugs can affect liver function. Do not continue the next dose without consulting the treating doctor first.
Next Steps
Please contact his treating chest physician or TB doctor immediately â do not give the next dose until they advise. Feel free to consult me directly on Practo for a detailed evaluation as well.
the development of generalized whole-body itching immediately following the first dose of AKURIT-4 (a fixed-dose combination for Tuberculosis) is a significant clinical event. This strongly points toward an acute drug-induced allergic reaction or hypersensitivity to one of the active anti-tubercular components (such as Rifampicin or Isoniazid), rather than a benign cutaneous response.
Next Steps
The history of being Hepatitis B positive, even if classified as inactive with previously normal Liver Function Tests (LFTs), introduces a higher level of clinical risk. Anti-tubercular medications in AKURIT-4 carry a well-documented risk of drug-induced liver injury (DILI), which can potentially cause bile salt accumulation manifesting as severe pruritus (itching) or even trigger a reactivation of the underlying viral hepatitis.
Health Tips
To clinically review your father's baseline pre-therapy liver function data, accurately evaluate the progression of his skin itching, and coordinate a safe anti-tubercular modification plan, please book a full consultation session via my Practo profile."
