Sigmoidoscopy done in 2018- ruled out polyps or other intestinal conditions.
Anal Fissures diagnosed in 2018
Internal Haemorrhoids(grade 1-painless bleeding) diagnosed in 2018
Present(June 2021) symptoms:
No pain since 2 years after following fibre rich diet and taking stool softeners.
Recurrent painless bleeding once in 1 or 2 months.(blood visible on stool)
Brief Summary:
I was suffering with painless bleeding after defecation. However, soon pulsating pain also started occuring in anal region. I was diagnosed with Internal Haemorrhoids and Anal Fissures in 2018. I religiously followed fibre rich diet and active lifestyle. After one year pulsating pain in anus subsides and I don't have any pain. However, once in a month or two I do notice painless bleeding. I recently visited a General Surgeon and he advised me for Laser surgery and Internal Anal Sphincterolysis. I asked for little cheap and conservative treatments which he said not possible. Please advice.

As per your whole clinical history of Anal fissure (painless bleeding)with diagnosed of Hemorrhoids grd 1which was diagnosed in 2018.
So what about right now?? I mean which grade do you have right now?
And if it's really if u have chronic Anal fissure that's really difficult to Heal by conservative treatment.
Even after that you can try b u medicine as well.because it's only 3 yrs of history...

You can try by conservative management apart of Fibre rich diet and stool softeners..there is other medicine are also u have to take thats going to heal or reduce the size of Hemorrhoids a D symptoms as well.
