As someone with a history of depression, ADHD and anxiety, I find it hard to maintain a relationship, I often fail to understand my partner deeply, lack effort from my part and often mess it up. What should be my strategy going forward to build meaningful relationships?

ADHD and anxiety are closely interconnected. Difficulty in maintaining close, long-term relationships too is a known symptom of ADHD. With your understanding (and probably accepting) your mental health condition, I feel, the ground for managing it is well-prepared. With this, there is a need for your partner to accept you in your present state of mental health and help you overcome most of your distressing symptoms. This leads to the requirement of understanding the ways to manage your condition in the best possible way. There is also a need for you to deal with close relationships with empathy, which you are often likely to miss in your relationship management skills. You are also likely to find it difficult to handle your anxieties and this may necessitate medication, sometimes. All these point towards the need for your seeking professional help both from a psychologist and a psychiatrist. Hence I would suggest you to begin doing so with a psychologist and proceed accordingly.
