What's the age? In general, if young patient, then to give her unrestricted life, surgery is required. Without surgery she might have to curtail down some of the routine activities to avoid further damage to knee structures like meniscus, cartilage etc...

I'm sorry to hear about your sister's injury. An ACL tear can be a serious injury and can lead to instability of the knee joint if left untreated. Surgery is often recommended to repair or reconstruct the torn ligament, especially for those who are active or participate in sports. However, depending on the severity of the tear, there may be some non-surgical options available. These may include physical therapy to strengthen the muscles around the knee and improve stability, as well as bracing or immobilization to protect the knee while it heals. It's important to discuss all of the available treatment options with your sister's doctor, including the potential risks and benefits of surgery, and make a decision based on what is best for her specific case.

The treatment for her injury depends on her symptoms of instability..Like if she has a stable knee and copes well with all demanding activities , she can very well be treated without surgery! Whereas if she feels her knee is unstable, she would benefit with sugery..
Next Steps
Clinical examination for instability in view of ACL and MCL injury..
