Unable to see the x-ray clearly but definitely there's rib fracture and collar bone to me seems fine. Ice the painful area if swelling persists While breathing do slow pursed lip breathing to avoid chest pain also guard the painful area with hands or pillow while coughing, sneezing or laughing Orthopaedic advice is important Analgesics prescription by orthopedic would help with pain and swelling For shoulder Physiotherapy would help and you can consult me for the same From what I could see on the x ray surgery seems less likely but orthopaedic doctors advice is highly recommended
