Since the patient is diabetic,he has to control his glucose level by Strictly following balanced diet and regular exercise is a must. If obese then has to loose weight,bp also should be monitored regularly for any fluctuations,advised to test his T3,T4 and TSH level,avoid stress, regular meditation advised and take plenty of fluids and fibre rich food, avoid junk foods.
Next Steps
Regular blood suger test is compulsory,along with LFT,RFT and lipid profile.

Get his CBC, fasting glucose level, hba1c, liver function tests, renal function tests, lipid profile, thyroid function tests, vitamin d and b 12, urine analysis, chest x-ray, ECG.Change in his voice is an indication for a ENT specialist consultation
