We have 4 month old baby , after feed baby latches off from breast , however stomach is not big , does that mean he is not full ?

No, the stomach does not need to be big after feed.
If the child has fed well, they will sleep for 1.5 to 2 hours, they will pass urine 8-10 times per day, they will be active and then will pass stools 5-6 times a day.
These are signs of a well fed baby.
