Hello, I am Dr. Romain Rajan, General Physician (Ex Govt Medical Officer at RML Hospital, New Delhi), and your case has been allotted to me. Thank you for sharing your blood pressure readings and medication details. I understand your concern and will explain this clearly. Based on what you have described, blood pressure readings around 130 to 135 over 80 to 85, occasionally settling to around 125 over 80 at night, while taking Telmisartan 40 mg, are generally acceptable and considered controlled. Current guidelines consider blood pressure below 140 over 90 as acceptable for most adults, especially if you are not experiencing symptoms like headache, dizziness, chest pain, or breathlessness. That said, the ideal target for long term heart and kidney protection is usually below 130 over 80, provided it is well tolerated. Why your BP may be slightly higher these days • Reduced physical activity during winter • Higher calorie and salt intake • Weight fluctuation • Stress or poor sleep These seasonal changes commonly cause mild BP variation. What you should do now • Continue Telmisartan 40 mg daily • Reduce salt intake and avoid processed foods • Increase indoor physical activity where possible • Maintain adequate hydration • Monitor BP at home and note average readings, not single values When to consult a doctor • If BP consistently stays above 140 over 90 • If readings cross 150 over 95 even once • If you develop symptoms like headache, dizziness, palpitations, or chest discomfort At present, your readings do not suggest poor control, and dose escalation is not immediately required if lifestyle correction is done. For personalised one to one consultation, medication fine tuning, or long term BP planning, you can reach out to me directly on WhatsApp at 85271646 seven seven. Wishing you good health. Dr. Romain Rajan General Physician

Do you have a pre diagnosed condition for which talmisartan 40 mg has been advised? As per the details given by you, 133/87 is the average BP reading which ranges till 125/80. This is not an emergency. You’ve nothing to worry about for now but you’ve to focus on lifestyle changes. Kindly take the medicine as advised. Even though, it’s winter. Try to eat healthy food, movement is important. If it’s greater than 140/90 despite repeated attempts, review the dosage with a doctor. Monitor the BP. If it’s greater than 160/100 with headache, blurred vision, chest pain etc. it is emergency.
Next Steps
Monitor your BP. Continue with similar medication
