The "period-like" pain you feel 1–2 days after IUI or natural attempts is common and usually due to uterine contractions. Prostaglandins: Natural semen (and even the process of passing a catheter during IUI) triggers the release of prostaglandins, which cause the uterus to contract and cramp. Ovulation Response: The hormonal shift during ovulation, combined with a sensitive cervix, often mimics early menstrual cramps. This does not necessarily mean the attempt has failed, but rather that your body is reacting to the procedure or the sample. Addressing "Unexplained Infertility" When tests like your hysteroscopy and tube checks are clear, the issue often lies at a microscopic level that standard imaging cannot see: Egg/Sperm Quality: Basic tests check for shape and movement, but not the genetic health of the cells. Implantation "Window": Your uterine lining may be healthy, but it might not be "receptive" at the exact time the embryo arrives.
Next Steps
Transition to IVF: Success rates for IUI drop significantly after 3–4 failed attempts. IVF allows doctors to monitor embryo quality directly, bypassing many "unexplained" hurdles. Laparoscopy: While your hysteroscopy looked inside the uterus, a laparoscopy checks the outside for "silent" endometriosis, which can cause both pain and infertility. Endometrial Receptivity Testing: Ask your doctor about an ERA test to see if your implantation window is slightly shifted.
