First consult with Dentist... Take an x-ray of particular tooth and confirm the diagnosis, whether there is a decay beneath the filling or there is infection on the periapical region of the particular tooth.... Once after confirming the diagnosis, the treatment plan should be discussed, whether the tooth should go for Root canal or Extraction... Before starting the treatment take prophylactic antibiotics and reduce your infection and then start with the treatment...
Next Steps
Already its been a severe infection of facial swelling, don't delay your treatment... Kindly consult with Dentist...

It is an infection . See a dentist as soon as possible because such infections can spread very quickly. You require antibiotics urgently and your doctor will prescribe after clinical examination
