Testis drying up

Asked for Male, 17 Years

My cousin is facing issues with his testis, suddenly one day he started observing a lot of pain in his testis, and now after some medivation doctor is suggesting to remove left side of it as it is drying up and might increase the infection if not being removed. So just wanted some suggestion if something is possible without the surgery. He might have developed this while sleeping in a wrong position oveenight.
