It can be hydrocele or hernia. If hydrocele( fluid collection) it doesn't need surgery till one year of age. Because sometimes the hydrocele settled on its own. If it persists at 1 year, then u can go for surgery. If it's hernia, it usually gets bigger when he cries or strains, then it reduces in size, when he sleeps. In this case, the intestine is coming out. So hernia needs early surgery to avoid complications. Both the diseases need only simple surgery without producing much scars. So u need not worry about it. It does not need any test or scan to identify. U can consult a nearby Pediatric surgeon. He/she can check with hands and tell u whether it's hydrocele or hernia.

Mostly it will be hydrocele (fluid collection) or a hernia. Better to do a scrotal USG, and consult your pediatrician. Hope there is no redness or pain.
