Swollen and Red
Hello Doctor, my son is 2.5 years old, Monday afternoon woke up with redness on penis top area. He got penis circumcision when he was 1 years old and March 2025 got hydrocele operation done We give him a bath daily and diapers are changed regularly. Yesterday I consulted the pediatrician attached a prescription but no relief. We applied ghee as well. He is crying a lot in pain. Please suggest which doctor we should consult for this condition and why this happened?

Thank you for your detailed message and for your attentive care of your son. I understand how distressing it is to see your child in pain. Based on your description—redness on the penile tip, pain, and no relief from initial medications and home remedies—there could be several reasons for your son’s symptoms, including a possible infection (balanitis), allergic reaction, or irritation, especially given his past circumcision and hydrocele surgery. My opinion: Given his pain, redness, and the lack of improvement, it is important to rule out any infection or surgical complication. Sometimes, moisture from diapers or use of non-sterile substances can irritate sensitive skin. Rarely, post-surgical adhesions or mild infections can develop even some time after procedures.
Next Steps
Stop applying ghee or any home remedies on the area, as these can sometimes worsen irritation or promote infection. Keep the area clean and dry—continue to change diapers frequently and consider using a mild, fragrance-free baby wipe or simply water and soft cloth. Monitor for other symptoms: If you notice fever, swelling, pus, or if the redness spreads, this could indicate a more serious infection that needs urgent attention. Photograph and note changes: If possible, keep a photo journal of the area (for changes in redness or swelling) to show your doctor. Consult a pediatric urologist or pediatric surgeon: These specialists are best equipped to handle post-surgical issues and genital symptoms in young children. If you cannot reach them quickly, visit your nearest pediatric ER if the pain is severe or if your child has difficulty urinating.
Health Tips
Allow some diaper-free time to keep the area dry. Use a mild baby barrier cream if there is skin irritation but no broken skin or open sores. Avoid any powders, strong soaps, or antiseptics on the affected area. If you have ongoing concerns or the baby’s symptoms worsen, please schedule a consultation so we can provide tailored care.
