Teeth is grossly decayed. Treatment could be Root canal treatment or extraction depending upon x ray finding.

This is an alveolar abscess due to pus accumulation from the carious molar tooth...if Extraction is must space maintainer is needed till the permanent premolar erupts at 12 years.. or pulpectomy can be done keeping the milk tooth as natural space maintainer

Its a dental abscess. It needs to be treated or else it will keep spreading and infecting new permanent teeth which are below milk teeth.
Root canal or extraction needs to be done depending on clinical examination and x ray.
If extraction is done, space maintainer should be applied.
It's a chronic alveolar abscess...there is a pus accumulation near root area, that's why he is feeling pain. And swelling because of pus.

The swelling and pain is due to the deep decay in the tooth.
That tooth has to stay their till he turns 12 or so before it gets replaced by a permanent tooth. So that tooth has to be saved. In case it can't be saved then removing the tooth and replacing it with a ring type appliance that is called a space maintainer which will help keep the space intact for the permanent tooth to come up is the next best option.
