You will require an Nt scan along with double marker at 11 to 13 weeks  and the cervical length need to be measured...If the length is less than 2.5 cm ,then a cervical stitch is required at 14 to 16 weeks and if adequate then may be injections 2 Wkly.... kindly discuss with your Gynecologist

Mac Donald  operation, at 14weeks of pregnancy, means cervical  stiching. But  before that you  should start treatment   .like folic acid and hormonal support.
