Last night my husband had some mid back pain and right side under rib area , so we went to hospital. They took ECG and said ST elevation, probable early repol pattern and borderline left axis deviation (-15, -29). Next they took 2D echo and it is normal.They took blood test troponin and it is also normla. they advised next to take angiogram to confirm thet there is no blockage in heart.is it really required doctor? He had no other symptoms except back pain. He took myospas tablet..and now there is no pain also..is it necessary to take angiogram?

If it is clear early repolarization pattern then u can avoid . You can repeat an echo to see any RWMA is there or not . If serial ECG and echo on follow up are ok it’s ok to avoid .

It has to be taken in a holistic manner..based on risk profile...if only early repolarization changes with normal echo and troponins, it's very unlikely to be CAD..but no test is confirmatory...hence we need to see the risk factors in patient like DM, hypertension, dyslipidemia, family history, smoking and subsequently plan exercise stress test like TMT..based on the outcome of these information,one can plan accordingly..
