Sperm comes only my hand

I am 33 years old. I got diabetes at the age of 27. I have been using it for more than 15 years masturbation. I use uphold tablet to increase erection. The problem MEANS i have been married for two years. Once the sperm does not come, it only comes if I do it by hand.How do I have physical intercourse and take out sperm.
