Repeat test
70 yo female, 48 kg, recent TSH 10, early cirrhosis, asymptomatic, prediabetes. Started on Thyronorm 50 mcg in this April. Taking sitagliptin M. After how many months and which tests should be rechecked for thyroid?? antiTPO has never been done. Is it required?

If goitre is new or growing, further evaluation is needed (e.g., ultrasound ± TPO). If goitre is stable, TPO is optional and not required for routine management. if u don't have neck swelling,the definately u don't require TPO no added benefit, or change in treatment based on tpo. routinely t3 check up 6 to 8 weekly. titration on antidiabetic required on the basis of fasting blood sugar and hba1c after every 3 month, fir more information pls connect.

Anti tpo is not required at this age . Moreover ,thyroid medication needs to be adjusted again . Because at 10 tsh level, this dose won’t be enough. Diabetes and thyroid are somehow related . Do consult and connect with me for further clarification..

Hi, Always recheck thyroid hormone levels after 4-6 weeks only. Anti TPO isn't a necessity. If you have a patient with chronic hypothyroidism without goitre, it's bound to be autoimmune only. The test won't change management This person needs a tailored diet and exercise program. That's very essential for general well being and dose reduction

What was her baseline TSH on which Thyronorm has started, at TSH of 10, generaly medicine is not recommended firstly for 70 years old female, ideal TSH should be repeated 6-8 weeks interval before starting Thyronorm. Even dose is high considering her age and body weight -generaly i start with 12.5-25 mcg daily and dose should be titrated every 2 month interval till target TSH acheived
