Hello, I understand your concern This is likely irritant contact dermatitis or diaper dermatitis, aggravated by: Prolonged overnight diaper use, Ammonia exposure from urine, Friction or moisture, Possible reaction to soap, wipes, or detergents

Stop diaper use at night temporarily if possible; use cotton underwear. Clean the area with plain lukewarm water; avoid soap or wet wipes. Pat dry gently—do not rub the area. Apply a barrier cream like: Zinc oxide cream (e.g., Sudocrem, Desitin) or Petroleum jelly (e.g., Vaseline) before sleep. Continue Fexofenadine for itching. If redness is significant or the child scratches frequently: Mild topical steroid like 1% Hydrocortisone ointment can be applied once or twice daily for 3–5 days under supervision. If there are signs of secondary infection (pus, foul smell, swelling), consult doctor for possible topical antibiotic. Avoid diapers whenever possible, especially during the day. Change diapers immediately after urination or stool. Avoid scented soaps, bubble baths, or antiseptics in the genital area. Ensure good fluid intake to dilute the urine.

Red flags: If symptoms worsen or persist beyond 5–7 days If there is painful urination, swelling, or discharge If itching disrupts sleep or causes behavioral changes

The problem seems to be less likely due to diaper rash. But we should rule out UTI first and does the child have habit of withholding urine , it is very common in this age group. The treatment given is conservative and correct. If no improvement or recurring, please get a urine routine done.

Use Candid ointment twice a day for a week to 10 days. Deworm - Syp Zenitel 1 dose at night today and repeat after 2 weeks ( a whole bottle is single dose) Please avoid nappy as much as possible - at least during the day time Expose the area to air

In case the healing doesn’t occur in a week - review at a paediatrician clinic
