Thank you for your query. As I understand your wife has had a PV leak at 22weeks 2 days.
There are 2 aspects that determine the interventions and your expectations from the health professionals.
1. The risk of infection to the mother.
2. Viability of the baby ex-utero.
1. In a PV leak all clinicians take care that the women doesn't develop chorioamnionitis which is an infective condition of the amniotic membranes.
She may have been given antibiotic prophylaxis for the same and monitoring would be continuing on a domiciary basis or after admission. She would have been given a sterile vulval pad to monitor the leak.
2. At 22 weeks, 444gm the baby hasn't crossed the threshold of viability ex utero meaning, the child may not have been developed enough in terms of lung function and neurological function to be able to sustain life outside the mothers womb without support.
Your wife must have gotten betamethasone injections to improve lung function.
3. There are two schools of thought in this regard:
1. Continuation of pregnancy till spontaneous onset of labour or on indication of fetal distress/ maternal evidence of chorioamnionitis. Some clinicians could consider amnioinfusion in this setting. Delivery mode for preterm breech would be by caesarean section followed by NICU stay.
2. Collective decision by a multidisciplinary team after determination of survival odds of baby to continue care to the mother and fetus till indication for induction of labour arises.
Next Steps
You and your wife shall be a part of whatever decision your treating gynaecologists offer, so you should have realistic information on the risk of cerebral palsy, neurological handicap and risk of respiratory distress, prolonged NICU stay, and long term complications in the extremely preterm newborn infant.
