Unplanned pregnancy means that you had sex for the sake of having sex but pregnancy just happened as a side effect . It is also the result of normal Vaginal intercourse. Regarding second part of your question, you should know what is  "safe intercourse" . For the details you may consult with me on Practo. There is no way you can prevent pregnancy 100% (except by total abstinence , i.e not having sexual intercourse at all) . Every method of Contraception has its failures.
