Pelvic pain 10days before Periods

Asked for Female, 27 Years

Every month, My fiancée experiences pelvic pain 10-12 days before her periods. The pain is very much intense than regular periods cramps. This pain only lasts for 2-3 days. Then after a week or so menstrual flow happens. Can any doctor here explain what it is? And how it can be treated? And how serious is it? I'll be really thankful to you. Please help. *Ps: we have visited couple of different doctors but no one actually has explained the cause of the problem, they have just prescribed painkillers. Ultrasound report is attached.
