Painful large blood clots during period

Hi, I have been diagnosed with PCOS, but my cycle has been the most regular it has ever been recently and this is the worst cycle of my life. I'm in horrible pain to the point where I sometimes can't move and have been bleeding twice as much with blood clots as large as silver dollars very frequently for six days. Do I need to go to an ER?
