hello mam the age of kid is just 5 years we may need a x-ray to go for a proper diagnosis basically we usually avoid extraction if the tooth can be saved that would be the best option so it's better to go for xray of the tooth first

It seems to be deep cavity in the posterior tooth as the child's age is 5 year only it's very important to get the posterior tooth to be treated because shedding will occur at the age of 10-12 years and posterior tooth is important for chewing as it affects the child's nutrition if it creates discomfort in eating.

Xray helps us to check how far the cavity has reached. If the tooth pulp (nerve) is involved, he will need a pulp therapy for those teeth. If not deep, filling will be sufficient.
Kindly get all of his cavities treated soon to prevent further spread, as he had entered into mixed dentition stage (milk teeth and permanent teeth co-exist) and it's easy for the cavities to spread to the newly erupted permanent ones.
