Hello Incomplete cleaning can also be one of the reasons for recurrence of symptoms. Progressive bone loss suggests urgent requirement for periodontal therapy.
Next Steps
Visit a periodontist and get the treatment to save teeth on time. As you ll proceed, bad breathe, plaque all such symptoms will resolve immediately. Ask your dentist to teach you correct brushing technique.I have seen on my follow ups, patients do maintain very well once they know what were they doing wrong.
Health Tips
Any age is too young to lose a tooth.

Hello sir, The Calculus and plaque deposits keep on happening. Doing cleaning once a year is must and also as per OPG, there is bone resorptions and it needs to be accessed to understand what is the underlying reason for it.  This will help further gaps in teeth and to the bad breath try 1) cleaning every year 2) improving the oral hygiene technique, use mouthwash and improved brushing method 3) sometimes the bad breath can be because of other health reasons like GERD, acidity etc. Do check with a gastro to rule those out.  Hope this was helpful
Next Steps
visit a dentist to understand the underlying cause.

Hello Calculus deposition keeps on happening in intervals of time. You can get scaling/ cleaning done once or twice a year depending on the calculus deposition. Opg xray shows gaps between teeth which is why calculus deposition can happen frequently . Consult with your nearest periodontist for further gum examination
