Nipple loose skin bleeding

Asked for Female, 20 Years

I've had a lump or a little skin attached to my nipple for over an year. When I went to the doctor they only said to apply lotion on it. However, it still dries and it grows yellow scab over it. It also bleeds when I scratch it sometimes. The skin seems to be coming from inside my nipple, and the piece of skin is loose outside the nipple. I really need to know how I can cure my nipple please help!
