This is a classical image of Anal Fistula. Usually there is swelling and pus collection. After some time it bursts and pus discharges; and pain is relieved. It appear that everything is fine. But after sometime again there will be accumulation of pus swelling and pain. This cycle will go on.
Usually in the patients of Anal Fistula surgery is advised. But it is not a good choice as there are chances of recurrence and other complications like stool incontinence etc. are very high. In this treatment after defining the fistulous tract by MRI or manual probing; Ksharsutra - a medicated thread is placed in fistula tract. Medicine present in thread debrides the tract and induces healing. This medicine works for 5-7 days. So it needs to be changed with a new thread at weekly interval. No. of Ksharsutra changes required for complete healing of Fistula depends on length, depth and branching pattern of Fistula.

Lump could be a single haemorrhoid or some sort of wart near anus.
