Your symptoms suggest chronic onychomycosis (fungal infection of the toenail). Since this infection has been present for more than 10 years, confirmation is important before starting oral antifungal medicines.
Next Steps
Please share a clear picture of the affected toenail to confirm whether it looks like a fungal infection. If needed, tests such as KOH mount or fungal culture of nail clipping may be advised
Before starting oral treatment, basic tests like Liver Function Test (LFT), fasting blood sugar, and HbA1c are recommended, as long-standing fungal infections can sometimes be associated with diabetes or lowered immunity.
Health Tips
Keep the feet clean and dry, trim nails regularly, and avoid tight footwear. Do not share nail cutters or footwear with others. contact me any time on Practo for further consultation.
Firstly, confirm the diagnosis of a fungal infection with a specialist. Once confirmed, oral terbinafine can be started. Proper nail trimming and hygiene are important factors to consider.

• In such chronic cases, oral antifungal medication is usually required for effective treatment.
• medicines are:
– Terbinafine 250 mg once daily for 12 weeks
• Baseline Liver Function Test (LFT) should be done before starting oral antifungal medicines because these drugs are metabolized in the liver.

Onychomycosis is a fungal infection of the nail. When the infection has been present for many years (12 years), it is considered chronic. In long-standing cases, the fungus can become deeply embedded in the nail plate and nail bed. Because the nail often becomes thick and damaged, topical treatments alone may not penetrate well and may not completely clear the infection.
Treatment requires time and patience, as improvement occurs only as the new healthy nail grows out, which may take 9–12 months for toenails.
For more query please share respected pictures. How many nails are affected ? What topical treatment did you use previously?
