Onychomycosis is a fungal infection of the nail. When the infection has been present for many years (12 years), it is considered chronic. In long-standing cases, the fungus can become deeply embedded in the nail plate and nail bed. Because the nail often becomes thick and damaged, topical treatments alone may not penetrate well and may not completely clear the infection.
Treatment requires time and patience, as improvement occurs only as the new healthy nail grows out, which may take 9–12 months for toenails.
For more query please share respected pictures. How many nails are affected ? What topical treatment did you use previously?
Next Steps
Combination of oral and topical treatment should be taken for 12-14 weeks. Nail trimming or Partial removal of nail might be necessary as nail must be thickened. Or consult a dermatologist for a laser therapy.
Rx
1. Tab. Terbinafine 250mg once daily for 12 weeks.
2. Topical Nail lacquer amorolfine 5% or ciclopirox olamine 8% for Local application.
(Or)

1. Tab. Fluconazole 300mg once weekly for 12 months
2. Topical Nail lacquer amorolfine 5% or ciclopirox olamine 8% for Local application.
DONOT TAKE BOTH TREATMENTS SIMULTANEOUSLY.
