The foreskin should be flexible and easy to pull back so you can clean under it when the penis is flaccid. You also need to be able to replace it to its original position without discomfort. If neither is possible and it is affecting the flow of urine or you are not able to clean underneath please see a urologist for help. The safest and most effective treatment is a circumcision.
