My father passed away last month natural death. I tried to contact doctors to check but every one busy in covid 2nd wave..and I did all formalities to him..for death certificate form 4A needed..but no one ready to sign the form..he admitted in southern railway hospital before 1wk of his death..I have negative certificate...CT scan copy..ECG discharge summary..now I need sign in form 4A for death certificate..can any doctors help me for past 20 days roaming for signature
