Muscle pain or bubbling, hand's nail goes blue in winter, weakness. Had consulted in AIIMS bhopal , all Tests are normal like ECG, BP . They prescribed vitamin b12, vitamin D . It works for 3 months since January. After April pain again started but not like earlier. Please advice...

Two things stand out here. First, the "injection/ant biting" type pain across the body is neuropathic in character — classic for B12 deficiency, which improved with supplementation and returned after stopping it. This means he likely needs ongoing B12 maintenance, not just a short course. Second, nails turning blue in winter is Raynaud's phenomenon — episodic vasospasm of the fingers triggered by cold. In a young person this is often primary (benign), but combined with widespread body pain and weakness, it's worth screening for an underlying connective tissue disorder.
Next Steps
Restart methylcobalamin B12 1500 mcg daily as maintenance. Get B12 and Vit D levels rechecked. Also get ANA, anti-dsDNA, CBC, ESR, and CRP to screen for connective tissue disease given the Raynaud's + diffuse pain combination. Consult a physician or rheumatologist.
