Multiple health issues..

Asked for Male, 25 Years

My father is 55 years old and is suffering from high diabetes , high blood pressure ,obesity ,once had a mild paralysis attack which reflect on his face , gastric issues , weak eyesight, and feels vacated (sunn in hindi) in his head and legs sometimes . Please guide me the steps to be taken for this multiple diseases . Which doctor to consult..?
