Some pain in the testes is OK, as it could be due to change in axis of the testes when it gets to our posture. Scrotum is a very loose bag and it contains testes in a separate individual sac for each. If you are getting pain at the time of gymming, it could be a sign of a impending or small hernia. But such pain can come at other times too like at lifting weights, laughing, straining at stools or passing urine.
Hernia is a externalisation of one body part into another, when there is a defect in the barrier separating the two.
In the intrauterine life ( before birth ) the testes are situated in the tummy and they are brought down in the scrotum in the final months of pregnancy. This external localisation of the testes is achieved through a small natural tunnel in the lower end of the tummy wall, which is situated near the fold where the leg and tummy meet ( groin ).

In some individuals who have chronic cough, or strain frequently at constipation or lift weights, the pressure in the tummy can open up this tunnel and it may cause the contents of the tummy namely the omentum and intestine to come out through it in the scrotum.
In mild cases there is ONLY pain and the herniated part goes back in the tummy. But if neglected the hernial sac can increase and slowly omentum and intestines can come to lie in it. This is called Inguinal hernia.
